Demographic Update Forms for Returning YISD Students

  • You are about to fill out the Online Demographics Update Form for students that are EXISTING AND/OR RETURNING TO YISD schools. In order to be eligible to fill out this form, the student you're enrolling MUST meet the following criteria:

    • Be an existing and/or returning student to any YISD School
    • Have attended a YISD School between 2012-2018.

     If you are unsure whether or not your student previously attended a YISD School, call your desired YISD School and they will be able to assist you with confirming which enrollment form you will need to fill out.

     

    REGISTRATION STEPS

     Step 1: Complete the Online Demographics Update Form

    To complete the Online Demographics Update Form for a returning student, the parent/guardian must have a Home Access Center login and password. Parents/guardians of returning students that were not enrolled in 2017-2018 will need to call or visit the desired YISD School to create a HAC account. Enrollment for the 2019-2020 school year opens on April 2, 2019 . Parents may log in to their HAC account and begin the registration process. Verify student information including date of birth, social security number, phone numbers, and addresses of parents/guardians. For best system performance, use Internet Explorer or Chrome.

    Step 2: Visit Campus

    After completion of the Online Demographics Update Form, parents/guardians will need to provide the following documents to their campus during the school's registration week:

    • Proof of residence
    • Updated student immunization records
    • Documents provided by the school

    Registration week is August 12 - August 16. Please contact the campus to obtain a schedule. Click here for YISD Schools.

    Returning student registration will not be finalized until these documents have been submitted to the campus.
