Evacuation Chair Requirements

If you have a student or staff member with limited mobility, you must:

1. Identify the individual and their primary locations throughout the day.

2. Assign a two-person evacuation team responsible for assisting the individual using the Evacusafe Chair.

3. Ensure the evacuation chair is stored near a stairwell for quick and safe access. 

Training & Certification

• Each team member must complete a 30-minute online training and sign an affidavit to be certified to operate the chair. 

• Training must be completed and affidavits submitted to the Office of Emergency Management.

Submission Instructions

1. After watching the video and reviewing the presentation, complete the affidavit.

2. Upload the signed affidavit to Safety Resources in the Google Drive.

3. Submit a copy to your Safety Coordinator.
